For 5th thru 8th grade students
Print and complete the registration form below and either bring it in to the Main Library, fax it to 532-8578, or email it to ddonahoo@shawls.lib.il.us. If space is available (laptops and digital cameras available for 10 students at a time) the Parental Consent Form, and the Contract for Participation will be sent to you with more complete information about the programs going on at that time.
